I just noticed something about subtitles on my system: there are some very very strangely named ones:

It's possible these have been around for a long time - I have no idea when this happened.  Any idea how to clean this up?

Would need specific examples but will guess improperly named external subtitles?

Thanks - that got me to them. There were some titles that had a subdirectory "Subs" in them and something about that was screwing things up. I think maybe "if there's a Subs directory and there's also a subtitle in the main dir".  That is just a guess.  Anyway - flattened all those out and the problem is gone.
