The work around I have been using is to use Dex on my note 20 ultra to cast Emby to the TV.  That seems to fix the issue.

The quality takes a small hit but it's not too bad.

Just to update my issue was caused Emby docker on the Xpenology setup.

The docker was causing the issues. I used the same files on the same server with Plex and Jellyfin and didnt have any issues with the system.

I have since moved off the xpenology software and using Unraid now.

I have this exact same issue.  EAC3 6ch audio causes lag and stutter issues.  Samsung tv 6 series from 2016, while the OP's tv is a 7 series Samsung from 2017.

Video is direct playing from a local emby server with good specs.

I wonder if this is a TV hardware issue.

How many subtitle streams does your media have?  At around 30 and beyond you will have problems, but possibly issues even with fewer than 30 on older models

Yes around 30.  Is there any patch that can fix this?  I don't even use the subtitles.

I don't believe so.  But similar discussion over here the other day.
