I've got several movies where chapter 1 and 2 are exactly the same at 00:00.  Anyone else run into this?

And what is chapter 3?

On one movie chapter 3 starts at 1:56.  On another chapter 3 starts at 9:53.  Are these determined by the movie file?

Yes they are. The chapters are inside the video file.
