What apps to lossless convert DVD, Blueray, ISO, and BDMV to mkv ?

Emby needs me to convert these.  Is it possible to do this lossless and fast?  What apps do this?  Thanks.

If there are many m2ts files in a BDMV, can they be quickly lossless joined into one m2ts?  Can emby deal with single m2ts?

I'm pretty sure it can convert losslessly, according to the webpage
