Remote Connection Fails After Working for Years

I can no longer connect remotely in Emby.  It has worked flawlessly for 2 years, but no more.  Nothing has changed on my end... same PC, same ISP/Router/IP., no recent windows updates.  It used to work without having to forward ports.  Canyouseeme.org now shows 8096 as connection timed out.  I have tried completely removing Emby and reinstalling, and relinking with my Emby Premiere/connect key but no success connecting remotely.  Have also rebooted PC and router.  Have also disabled and then uninstalled my 3rd party antivirus Norton 360 in case it was a firewall issue within Norton, but no luck.  Once I uninstalled I also made sure Windows Defender firewall settings allowed access, and then turned off automatic port mapping in Emby and turned back on.  Lastly, i disabled UPNP and re-enabled on router.. No luck

Just a router port forwarding misconfiguration.
I'm going to add a section for Verizon FIOS to our remote setup KB article because Verizon makes it more confusing than needed.

He's up and running.

It probably wouldn't hurt to grab some screen shots (with permission) from different people I help remotely to build up a few pages of different routers with basic port forwarding for Emby.
I've got a few brands I own I could document as well.

Steps:

Login to Fios Router

Go to Firewall---> Port Forwarding

Select your server IP from the menu

Select Custom Port

Port 8096

Click Add

Apply

It's actually quite simple but the way they show it is bad and confusing in the list of ports.  You also can't look at settings or edit things so it's not intuitive trying different ways of setting this up.

But its easy when you see me do it. :) 
I learned the hard way too. LOL

I'm also using Verizon FiOS as my ISP. While Emby works fine within the LAN from a remote system I get an error "Refused to connect." 

From Advanced > Port Forwarding Rules I added a protocol for Emby, then from Main > Port Forwarding added it.

That doesn't look right. Remove that one you just added then try this.

Select the IP/machine from the first drop down.  Then choose custom in the 2nd dropdown.

Then enter either 8096 or 8920 depending if you're setting up non-ssl or ssl connection.

Click Add..
