TV Season not displaying all eposodes added

Have an odd issue where a recent addition of a season and some episodes to a TV show is not listing the episodes in the UI.

Please advise on the next steps to get this issue resolved, thank you!

Notes:

  • when creating the new season sub-folder "season 03" under the TV series "Teen Titans" it populates only the 7th episode, and not the other 1-6.
  • when it scans the library it creates NFO files for the season and all seven (7) episodes.
  • season 02 for the same TV series does not have this issue nor do any of the other TV series
  • TV series are in a dedicated drive/share with best practices folder structure.
    • the dedicated library content type is set to "TV Shows"
  • issues seen from multiple systems, browsers, and emby theater
  • Emby windows server version: 4.5.4.0

What I have done:

  • ran "Scan library files" for the season
  • ran "Scan library files" for the series
  • purged the .nfo files and ran "Scan library files" for the season
  • copied the files to a backup path, deleted from emby. copied the season back, and ran "Scan library files" for the series.
  • rebooted server
  • ran "Scan library files" for the TV Shows library

Try running Scheduled Task 'Scan media library'.

looks like it was getting confused with season 02 for some reason, as I noticed season 03 EP 07 was showing as the season 02 EP 07 name.  Moved files out, purged the whole series, moved it back with out any NFO files, forced a scan of the library and now I am back in business with it showing everything properly.
