If I manually edit fields, is that prevented by the lock, or does it just affect a refresh? What are the circumstances in which one would want to refresh things? How do Tags fit into this system, since I'm not aware of them ever being automatically generated?

It only prevents the automated metadata fetch from reverting your manual changes. This applies to any metadata that has the feature that can be changed by metadata fetching. The reason to refresh things varies, but generally you do not need to refresh unless you've lost metadata or incorrectly changed metadata. Tags, AFAIK, have no automated data fetcher.

Keep in mind none of the locks apply if you IDENTIFY a movie/episode as that is basically saying the meta-data info is wrong and DOES need to be reloaded. UNLOCK protects against refreshes.
