Odd metadata image for a specific file

I have a specific video (it's one of my kid's cartoons) and it gets this weird placeholder for the primary image.  See attached file.

 

It is only this particular video and I can edit the images and change it to the correct image but everytime the library is scanned the selected image is replaced with the "Your Watchlist tracks the movies you want to see" image.

 

Any idea why this is happening and how to make it stop?

What externalids appear for this Video (edit metadata on the video)?

Sounds like you have that image in your media folder next to your video file? is that correct?
