Lost my surround sound and audio tab is way off

So I noticed that my surround sound no longer was working and when I went to check the audio settings it didn't have any of the audio passthrough checked. When I tried to select them the screen basically just split, top layer and a bottom layer. I cant select anything and have to close the app and reopen it to get the top layer off the screen.
