Inconsistent manual sync results, not picking up a replaced episode on one installation

I've found Emby for Kodi's syncing to be really excellent, except for one odd problem.

 

Sometimes, when an episode gets replaced on my system (say, a 720p version being replaced by a 1080p version), the new episode is not added to my Kodi database on my main HTPC.

 

Curiously, Emby Server IS finding the new episode - I can play it via app or web - and it is getting picked up by a sync with my other HTPC(OpenELEC on an Odroid C1).  It's just not being detected by one HTPC (nVidia Shield, SPMC).

 

The original episode was added, but Kodi could not find it when my wife tried to play it.  She removed the database entry when Kodi asked.  Now, however, it won't add that episode back.

 

 

The new version has the same season/episode number (or, in this particular case, date) as the old one, but the filename is different as the quality identifier in the filename differs.  

 

 

What can cause Emby for Kodi to not add an existing file to the library?  

 

I'm suspicious it has something to do with the wife having chosen to remove the entry from the library - does that tell Emby to stop adding that episode in the future?   Can I disable that?

I should note: I'm pretty sure a database reset will correct this, given that the episode in question shows up in Emby directly and in other Kodi installations.  I'm just trying to figure out what could have caused the problem, as my library is fairly immense and a full reset is extremely time consuming so not something I want to do regularly.

Are you using the fast sync server plugin?

Is you go into the Kofi ad on and do a manual sync?

No, no plugins at all.

 

And yeah, going into the Kodi add-on (settings>add-ons>video add-ons>Emby) and doing a manual sync doesn't pick up the replaced episode. It does get any other new stuff, but not replaced episodes.

 

I did a full database reset and that worked.

 

I suspect that it's because my wife removed the (Missing) episode from the Kodi database, as it doesn't appear to be a problem otherwise. It's very strange though, going to have to experiment to see if I can reliably replicate the behavior.
