Reset Password of Admin account wiped all user passwords

Long story short, we had an issue where our admin account password was not working on multiple clients. We followed the reset password for user procedure where the server generated a passwordreset text file with a 4 digit PIN. After following the link and entering the PIN, the server proceeded to wipe all users passwords instead of just the one account.

 

Is this by design or is it a glitch?

Do you have the log files from when this happend? If this turns out to not be by design i'm betting someone will want to look through that.
