Live TV Channels page is broken

Emby Server > Live TV > Channels won't allow you to configure a channel - the guide fields are missing.  AND, for a bonus, you get a spinner while trying to work on the page!

click a channel radio button to enable it, click the channel to open the Edit dlg - observe the incorrect form is shown and a spinner that persists even after dismissing the dlg.
