Show not recording

Note historically recording shows is hit or miss compared to Emby's WMC counterpart however last night recorded CNN Have I Got News For You.  I have a thumbnail on the screen when I look at the web interface but no shows.  If I look in the folder it says it recorded it to I see this:

For Activities.  Not sure the two entries and the colors.  Seems like from the times the bottom one when it started and the top when it stopped?  Clicking the (i) icon I see:

Not sure if this is a path issue as it's very verbose the file naming or what.  The face that there are those same named other files in the folder and I can create a folder in the Season 4 folder with the same name makes me think that's not the issue.
