Title is taken from Folder Name instead of filename

I have a few titles that are taken from the Filepath instead of the Filename. Why does this happen?

Hi, what are the contents of the folder?

From a single item in folder standpoint folder name is used, if more than one item exits item names are used.

But preferred method is every movie has its own folder.
