Some thumbnails missing after 4.9.1.80 update

After updating the server to 4.9.1.80 many of my thumbnails went away, but it's very inconsistent so hard to troubleshoot. 

I found the logs saying permission denied errors so I changed permissions on ./metadata/temp to 777 my live TV tuner/guide pictures all load.  That's good but seems like a terrible idea. 

Wierdly though the movies loading is very intermittent. You can see some movies load and some don't.  Example How to Train Your Dragon vs 88 minutes. Both have same permissions and group and owner all the way down but 88 minutes works while How to Train Your Dragon doesn't. 

I can get most of these to work by chmod'ing 777 more metadata folders but I don't really care for that. And for some reason there are so a few TV recording hold outs that don't work. 

What else could be leading to this error so I can go back to a safer permission structure of 755 on directories and 644 on files?

Here is a log for before I fixed it but I think it was a few days after I upgraded.
