that can happen if for example user A has already logged from a Web browser and his/her credentials are cached (cookie), even the new policy applies, the cache on the browser skips the policy, and continue logging him/her from a now unauthorized device.

try clean browser cache and cookies, or even better, if you have a phone or tablet try to login with a user that has no permissions to log from this device and see what happens.
