Notifications not working reliably

I have notifications set up for Added and Removed media, and playback events.

These worked fine, but the Added and Removed media events are having trouble since I've been in the process of replacing entire series with higher resolution files.
The first few series I've replaced were notified fine; the webhook received the removal and addition of every episode just fine, for many seasons. (Only nitpick I have here is that the addition of the new episode is reported before the removal of the old episode. I would expect the other way around.)

But lately, these notifications are sparse, almost, non-existent. I just replaced 11 seasons of The Walking Dead, and JUST ONE episode got reported after the library scan (both for addition and removal). First I thought the endpoint had trouble processing the requests, but the emby logs also shows just 2 requests to the webhook at that point.

Did I somehow overload the notification system?

Playback events still work fine.

Running 4.8.8.0

Okay, here's a breakdown of what happened. I've set up a webhook script through 'notifications' to log changes in my library and playback info. This worked fine until I started making big changes to the library; I've been replacing my 720p series with 1080p versions. I've overwritten the old files with the updates ones, kept the filenames.
The library itself updates fine, the notifications that should follow behave erratically. For every episode I expect 2 entries; one for removing (the old), one for adding (the new). It is notable that for episodes that DO get notifications, I always get both 'removed' and 'added', so at least that is consistent.

Here's a summary of what happened

May 31 (first big changes):

* "The Office (US)": replaced 9 seasons, 185 episodes
- NO ISSUES: Notifications for every episode (one for adding, one for removing)

- "The Good Place": replaced 4 seasons, 50 episodes
- NO ISSUES: Notifications for every episode (one for adding, one for removing)

June 3:

* "The Bridge (2011)": replaced 4 seasons, 38 episodes
- NO ISSUES: Notifications for every episode (one for adding, one for removing)

June 4:

* "His Dark Materials": replaced 3 seasons, 23 episodes
- ISSUE: No notifications for season 1 episodes 1 to 4
- ISSUE: All other episodes were reported TWICE (two for adding, two for removing)

* "The Dark Crystal: Age Of Resistance": replaced 1 season, 10 episodes
- ISSUE: Notifications ONLY for episode 6 and 7 (one for adding, one for removing)

June 9:

* "Rick and Morty": replaced 7 seasons, 71 episodes
- ISSUE: No notifications for all of season 1, season 2, season 6 and season 7
- ISSUE: Season 3 only first episode (one for adding, one for removing)
- ISSUE: Season 4 only episodes 6 to 10 (one for adding, one for removing)
- Season 5 was the only season reported in full (one for adding, one for removing)

July 5:

* "Family Guy": replaced 20 seasons, 387 episodes
- To summarize: First 5 seasons NO ISSUE, from there on lots of missed episodes, notifications for last 3 seasons missing completely

* "American Dad!": replaced 20 seasons, 366 episodes
- ONLY 6 episodes out of 366 got reported.

I could go on, I've replaced many more since then, not one of them getting all the notifications they should have (up to "The Walking Dead" that I replaced yesterday, getting just 1 episode notification out of 177 eps)

I've attached a log of yesterday. Look for 'emby.php' where my webhook is called. There are a few playback notifications in there too (those still work fine)

So what is the problem from yesterday?

Like I've stated clearly: I've replaced the entire 'The Walking Dead' series (177 episodes) that day, and received notifications for just ONE episode, where I expected notifications for ALL of them. My breakdown clearly shows degrading reliability over time - starting with perfect notifications for "The Office (US)" and "The Good Place", and progressively worsening with each new batch, up to a point where almost none of the changes are being reported to the webhook.

replaced how? What exactly did you do?

Look, I've explained everything I can in the OP and the follow-up. It's all there in detail. I have no idea how I can explain it any clearer.
