Both. I noticed it with the boxes that were online at first. Yesterday I saw it in my theater which was off before.

Mostly realtime sync. ie kodi was online. Yesterday I turned on the theater box and saw it there too.
tbh I did not specifically test offline



Edit: Yesterday it could be that the library scan was long

Was it a new artist, or an existing artist (only an addition video)?

New artist, not already in the database. Existing artists does not have that problem. 
BTW I usually add an artist.nfo with the artists

I ran several tests. So far I wasn't able to trigger the issue.

Interesting. Just curious,What is purpose of the no info tag? When is it used?

--NO INFO-- is a placeholder when "critical" metadata are missing, like artist or album (for music/musicvideo). 

Reason: Kodi has some staggered nodes for music content (but not limited music content only). e.g. Artist node. The artist node has artist -> album -> song. If the content of an artist has no album(s) info just songs, the node view would never list the any songs of that artist. Therefore I use placeholders to keep the browsing consistent.

For your sync issue: I assume emby server did not report the artist info of the musicvideo at the time when the new content was reported to Kodi/plugin. Therefore the placeholder was added. Sometimes Emby adds additional metadata after new content was reported and never reported an update of the affected content. I tried to force this issue with musicvidoe content, but had no luck. I tag my musicvidoe content with musicbrainz/Picard (Picard can handle mp4 videos as well). Therefore it has the artistinfo embedded in the mp4 metadata. When I scan the new content with Emby, if has all the infos and reports correctly to the plugin. In your case I assume, the artistinfo was added (by Emby) at a later point after the new content was already reported to the plugin, and the updated metadata are never reported to the plugin. However this is speculation, as I was not able to trigger the issue myself.

If it not too much work I would suggest using descriptive placeholders, no-artist-info, no-album-info etc to provide the user with a clue as to what is happening.

I've read a few posts talking about Kodi and the inability for it to handle a movie in multiple boxsets/collections. I enabled the Boxsets as tags, but how do I put it to use? How can I show all my boxsets and have a movie be visible if it is in multiple sets?

I have multiple Kodi profiles corresponding to my multiple Emby users. I've added the users in the E4K add-on but how do I link the Kodi profile to its respective Emby user?

There is a custom node "collections" in the "video add-ons"/"Emby for Kodi Next gen (Video)". You can use it to customize the skin.

Probably also disable "Media" -> "Video" -> "Show movies sets"

Thank you. I'll see if what you mention is different than the Emby Dynamic Collections I've been using. I'm only showing a few movies, sometimes zero, in my collections where previously they were all populated correctly. 

Additional question:
