Is it time to revise "How to Report a Problem"?

I've glanced at the post a few times but never stopped to read the whole thing and that should tell you something. I have a feeling that many others do the same. The original was posted in 2013 and I don't see edit history for it.

My thoughts are that it could be revised to a simple template format to ask for the important info, including system details which the original doesn't mention. The guidance for collecting logs can be pushed to the bottom and also revised to be more concise.

Maybe some changes will help in getting people to actually post the info when requested.

Yea I did edit one of them recently but unfortunately there are multiple and they don’t stay in sync automatically
