Movie Extras in Playlists no longer showing?

Running 4.8.11.0

I was working on updating my playlist and thought something was missing so I went to re-add it (even though I was sure it was always there) and it gave me the "this already is in the playlist, add again or skip" option, so the item was detected in the playlist but not showing. Confused, I also checked the m3u file and found it (both the original and when I added again at the bottom) even though neither copy shows in the web interface. So I created a new playlist with only 2 items to test. When I added the movie extra as item 2, the auto-generated icon changed from 4 copies of the same cover to 2 and 2, so it saw the item and read its picture, yet in the web interface it says 1 track and only shows 1 item?! I can mix movies and tv together no problem but movie extras seem to all disappear.

I already did up above in the original post with step-by-step of how I created a new playlist that shows the problem and a copy of the m3u file. I repeated the exact same steps post-upgrade and ran into the same problem. I don't believe any of the logs will be helpful since there are no errors or playback problems but I can check.

OK I ask because it seems to be working just fine in my testing.

Just to make sure there's no caching or problem with leftover files not being cleaned up, I went with completely different files and a new playlist name and still seeing the problem. I don't even have to test as far to see the problem. This time I did it backwards, I found an Extra in a movie (\\whshawk\mcu movies\Thor, Ragnarok (2017)\Extras\Team Darryl.mp4) and clicked add to playlist and created a new playlist called "new test." Then went to my Playlists and see the new playlist added and it's using the same thumbnail the extra shows on the movie screen (x4) and has NO contents to the playlist that can be played. 

If I then add a movie to the playlist, I get thumbnails updated 2x2 but only one item in the list:

At first I thought maybe the issue was length of video, but I just so happened to pick a "movie" (short) that was under 6 minutes and it showed in the playlist vs the extra that is over 6 minutes and doesn't.

I don't see anything odd in the logs at time of creation:

Oh, and just for fun I was curious what the playlist Play button would do when only the extra was in the playlist (so no list showing below) and got this message:
