Episodes are combined, in a cascading loop.

I think I know why, Is it because of the

#1 name - S01E08 

That will combine episode 1 through 2 then 3 then 4 etc.

Yeah, funny one. 

I'd just remove show title from episode filenames, that would likely sort it out.

I can also see that TVDB has some issues with it.

Only S01-E01 has a air date

Not only TVDB, it's still incomplete with the other online providers as well (Episode titles, thumbnails, overviews...), guess it'll be updated over time.
