How to do custom metadata fields/groups?

How do you set up custom field groups in the metadata manager? For example, let's say I wanted to add a "Themes" category between "Genres" and "People". What do I do to add this so I don't have a bunch of random tags/metadata where they don't make sense?

Hi, you can't create a new field, but what you can do is use Tags to tag media and create custom groupings.

You can also use the collections feature and add items to your own Collections.
