Photos shown don't match preview

I'm using Version 4.7.14.0 on a Synology DS918+ (x86 arch) - it's overall working great!

We're indexing some ~20k home photos. I got indexing working alright and all previews work great and all the previews do match the actual content of the file when verified manually.

However, going to Dashboard -> Home Videos & Photos -> Photos it's permanently the case that certain (not all) photos, when you click on a preview, its opens a different image that comes from an entirely different date and time and source folder. 

This is persistent behavior. I tried the following as a remedy

- rescanning metadata & replacing images (on the individual image) - no effect
- rescanning metadata & replacing images (on the album) - no effect
- re-scanning library files
- restarting diskstation
- restarting emby server

I'm filing this under "Synology" as I can reproduce this using both the web interface as well as the iOS client.
