Custom intro/Cinema intro not working on TV shows

Sometimes it seems like Cinema Intros only works with the first episode of a series. Then the first episode of a season, then the latest episode of a season when the rest is marked as played. But even that is unreliable. can someone describe the intended functionality? I've tried with Premier on the web client, Android (not AndroidTV), and Roku with the latest stable builds. For Movies, the trailers and customs work just fine EVERY TIME but episodes are hit and miss. I understand that any episode auto-played in a playlist or up-next in the series/season would not have any pre-roll (even though that seems like it could easily be built into VP or Cinema Intros) but why is it so hard to pin down behavior for items that are launched directly from the menu? Am I the only one who is experiencing this still? I just want every user who selects any episode from the menu to be forced to watch old, camp, Halloween commercials before they get what they want. Am I missing something?
