continuous playback issues

I am continuously experiencing playback issues.   I use the web client and Windows 8.1 store app.  Issues occur on both clients.  I have attached logs with latest video that keeps freezing.  I've also attached screenshots showing all the issues I see on daily basis.

 

Can you provide troubleshooting steps that helps us isolate the issue?
