What is causing this error?

Good morning, Emby.

Moving some items around in my backend, and ended up re-adding my entry for MythBusters into Emby. As it scans, it gets 99% of the episodes scanned in properly, and displays ALL episodes as they should be displayed, but also adds S08E05 as a separate entry as well. Any idea what has caused this?

Any idea what could be causing this? Attached is a screenshot and a server log.

There is no duplicate file, before this is mentioned.
