MKV movie chapters not visible in any player but work in VLC and MPC-HC

System setup:

• Windows 8.1
• Media Browser Theatre 3.0.5214.16310
  • Hardware Acceleration Mode: DXVA2CopyBack
  • Audio Bitstreaming Mode: HDMI
  • Enable Reclock: Disabled
  • Enable madVR: Enabled
  • Enable XySubFilter: Disabled
• Media Browser Classic 3.0.163.0 5-3.2
  • External Player: MPC-HC 1.7.5 with madVR
  • ISO Mounter: DAEMON Tools Lite 4.48.1.0347

Issue:

• Looper (2012) MKV (converted using DVDFab with MKV passthrough profile) does not show chapters
• MBT - Movie plays, there is no option to select a chapter when clicking *
  It is still possible to scan using the mouse on the seek bar or the scan forward/back buttons.
  The play state does not save for resuming
• MB Classic - The chapters are also not viewable in the GUI
  Playing the movie the chapters can be selected using MPC-HC
  The play state does not save for resuming
• Web Player - Movie plays but the seek bar is greyed out (not clickable)

Testing:

• Re-ripped movie and converted movie, same result
• ISO works fine, chapters are visible and usable in MBS, MBC and Web Player
• Movie chapters show up fine in MPC-HC and VLC
• I have ripped over 300 movies using this method, I haven't come across this issue until now:
  • Rip to ISO using SlySoft AnyDVD HD
  • Convert to MKV using DVDFab
