90% sure you are trying to start the server with a hung instance of ffmpeg running.

To my knowledge, the issue has not been resolved.

 

The server will crash (with odd errors relating to the ports or the first thing that tries to access the library) if an ffmpeg instance we started is running when we attempt to start.  The server does not currently kill off any rogue ffmpeg processes when it shuts down (this seems like something we should do but perhaps it isn't that simple).

 

The only thing that was resolved was that there were some cases with the new throttling that would cause an ffmpeg process to hang.  Those were fixed but there are still plenty of possibilities for ffmpeg to be left running when it shouldn't and those will cause a problem on re-start 100% of the time.
