I'm getting numerous errors related to the metadata, i'm not sure if this is the reason why my primary images are randomly vanishing but maybe this will provide some info.  Let me know if more info is needed.

UPDATE:
It seems the issue has been resolved.  I reset the metadata folder permissions in windows to allow everyone full control plus turned off windows folder indexing for that folder.  Windows must have automatically changed the settings because I did not mess with any of the permissions myself.
