Having Issues with Playback related to metadata

OK so every now and then when I go to play an episode of One Piece on emby it fails to load and I get an error (see screenshot).

I am able to resolve the problem by refreshing and replacing all metadata, however the problem has popped back up at least 3 times so far.  Any idea what is happening and what I can do about it?  Could this be an indication of data corruption on my storage media or something like that?

 

So far the issue has only ever cropped up with this one series (but atm its def the series I have been playing most (but not exclusively) on the server.

So put the info in a different place and include server logs?  Ok next time it happens I'll pull logs and remake the post in the right place.

I am not sure if this is related but this seems to happen when I have 2-3 versions of an episode.  If i move the extra versions to a folder with a .ignore file and refresh the metadata it seems to resolve.  Thats fine I have a way to fix it but would be nice for the diff file version thing to work without issue.
