Can’t add second device

How are you attempting to log in? Emby Connect or with manual login?

step 1) You might be able to fix this if you Reset the settings on that device. This will clear the registry for Emby on just that specific device where you do this. If you cannot login you may not be able to see the option shown above. In which case you can delete all Emby apps from your Roku and restart the Roku. That will effectively delete the registry.

step 2) Afterwards go to your devices tab of your Emby server and delete all the Roku devices you find. Do not worry they will regenerate when that device is used again. These are just used to store authentication.

Once you clear the settings on the Roku app and have removed all Roku devices now. You may have to reinstall Emby at this point on your Roku if you had to delete it in step 1 above. Then restart the Emby Roku app. Logging in should work without authentication errors if your username and password are correct.
