Episodes: Missing/Not Missing

Recently I've had this weird issue where episodes are being reported as missing right next to the episode in question.  It doesn't happen for every show, it just randomly does this.  See below screen shot for example.  The episode underneath plays just fine.  Refreshing metadata does work to fix the error most of the time, but i have to do it several times sometimes.  Any ideas why this is happening?
