Prior to server version 4.10.0.40, I had only ever had the issue on the TestFlight version of the iOS app. I don't see any specific pattern to cause the issue, it just seems to happen about 75% of the time I go to the dashboard.

It seems to also happen occasionally at random, but that could just be a real time scan I did not see happen. This behavior so far has been repeatable in both the TestFlight iOS app and Web app.
