Hardware Acceleration / Transcoding always falls back to CPU

I have tried a variety of settings and have had no success getting HWA working. It has been a while since I tried to get it working, so I would like to start from scratch, and get some others' input.
The failure seems to apply to any conversion or transcoding of video (including PGS subtitles), falling back to software encoding and maxing out my CPU. I've attached some logs here.

I forgot that I had posted previously. My other post might still be relevant. Regardless, it contains more hardware information.
