cover art custom overlay naming convention

I'm wondering if there is a resource file that shows us the naming conventions and image sizes to use when creating custom art. 

for example I want to create a custom subtitle icon for English. I have created one and named it subt_en.png but it doesn't seem to work, should it be named subt_eng.png?

can i have an icon for Und subtitles eg subt_Und.png or a default for unknown subtitles

also, are we limited to file image size, for example 100x100 pixels
