Episode images not correct

I've recently noticed what seems to be a problem with episode images.

 

On my recorded TV library, for Episode Image Fetchers, I have first theTVDB, and second, Screen Grabber.  When there is an episode that has no episode image on theTVDB, it ends up showing the season poster in place of the episode image.  Here's a screenshot example:

You can see the last episode shows the poster.  Now, I've checked on theTVDB.com, this only happens when there is no image available for this episode on theTVDB.  But if I click on the three dots, choose "Refresh Metadata," do not select "Replace Existing Images" and hit OK, it'll replace this poster with a screen shot.  See below:

So it must recognize that theTVDB has no image and it uses the second choice on the list and falls back to Screen Grabber to just grab a frame from the recording.  That is what I would expect it to do. 

 

But my question is, why doesn't it do this when the episode is first recorded?  Why do I get the poster?  With no image available on theTVDB, shouldn't it always fall back to Screen Grabber, I don't get why I have to manually refresh metadata for this to happen?

 

And here's a bigger puzzle.  If I uncheck theTVDB from the available Episode Image Fetchers and make Screen Grabber be first and be the only thing selected, then I always get the poster.  But again, if I manually refresh the metadata for an episode, it grabs an image just fine...

 

Also, I have a scheduled task to "Scan Media Library" that runs every 12 hours.  The description says it "Scans your media library and refreshes metadata based on configuration."  So why doesn't this refresh the image?  I've had episodes remain there a week still showing the season poster until I manually refresh that episode metadata.

 

Sorry, that's a lot of questions in one post.  Any ideas?

Yes, reproduces every time.  Several more episodes were recorded last night.  For an experiment, I had unchecked theTVDB and left only Screen Grabber checked and moved to first in the list for Episode Images.  All four of the episodes recorded last night came back with no screen grab, showing the season poster image.  Without changing the Episode Images settings I went in to one of those eposides and chose to refresh metadata and the screen grabber image appeared.  But it never works automatically after the recording, only if I go manually refresh.

 

With theTVDB set as the first service to use for episode images it works great if they have an image.  But if they do not have an image for the episode then I see the same behavior.  Never grabs an image after the recording but always works with a manual refresh.

Ok so this is only on recorded episodes? @@Happy2Play can you reproduce with a new episode?

I can't reproduce adding a new episode that does not have a image on provider site and screen grabber not generating one upon import.

I just recently switched from Plex to Emby (like 6 weeks ago maybe) and I had a lot of TV episodes already recorded.  I couldn't say with 100% certainty, but I do not recall that any of them were missing their episode images when I added them to my Emby library.  I'm sure some of them would not have had images on theTVDB, so that means Image Grabber worked fine for episodes that were already recorded and just added to a library.  That seems consistent with Happy2Play's result as well.

 

But when a new episode is recorded (using a Hauppauge card from my antenna), Image Grabber does not work for me, this is 100% reproducible.  It'll work after the recording is complete if I do a manual metadata update, but never works automatically after the recording completes.  And it does not update when the "Scan Media Library" scheduled task runs.

 

I'm happy to send logs (if you could point me to instruction on where to find them), or help with more examples, or whatever else I can do to troubleshoot.
