I just installed it and I am very pleased with it.  With Movies, would access to the Specials folder be provided by the Skin or the XBMB3C Plugin?

The only way I know to do this is make the folder a [Collection].  Then put the movie and special under it.

This is not something I use.  If it works differently in the MB3 web client let us know.

In the MB3 Web Client, items in the "Specials" sub-folder of a Movie folder show up as "Special Features" like this

This is documented in the MB3 KB about Media Folders here http://mediabrowser.tv/community/index.php?/topic/674-media-files-folders-structure/#Movies

Special Features
Special features for movies can be stored in a specials folder under movie folders. Nested folders are not supported.

/Movies
/Home Alone (1990)
   /Home Alone (1990).mkv
   /specials
      deleted-scenes.mkv

XBMC gets this sort of information with a separate plugin "Video Extras", from what I have noticed MB3 seems to include "Specials" with TV Shows in the Season folder automatically but Movies are treated differently.

How does 'Video Extras' communicate with the skins?

Some info I found.  I have not evaluated it so it's just a jumping off point.

Probably quite similar to what we are going to need to do for Actors...

All I know is that you can integrate it into skins similar to how tvtunes etc is integrated...in my 1080++ skin you can access "extras" via the info screen. If it finds something to be shown a separate dialog appears where you can play the extra...Not really integrated but it works. I don't think this will be an easy integration in this add-on. Maybe we could provide an extra property with a movie stating there are extras available and then from the info screen of xbmc call a separate function to show a list of extras in a sort of similar way to how the search dialog presents results?? Just thinking out loud here :)

Yep - that is exactly how it would have to work.  Do you think Actors would be the same way?
